We want to hear from children and young people, and their parents and carers, about Education, Health and Care Plans. We’re most interested in hearing about:

We want to make sure that every child, young person, and family is getting the right help, at the right time. That’s why we’ve co-created feedback forms with our Parent Carer Forum, Southwark Independent Voice.

Young people and parents/carers can tell us what’s working, and what needs to change, using the feedback forms below.

We have also produced posters to tell families about the feedback forms. Please use these in your schools and other educational settings.

Another way of sharing the forms is when you're writing a letter to parents and carers – we've written some draft text you could use.

 

Southwark Council wants feedback on your experience of:

  • Having a needs assessment (asking for an EHCP, and seeing if one is needed)
  • Annual Reviews (a yearly check of what’s on the plan and if it’s working)

Please be honest about what’s working, and what needs to change, so that the SEND Service can respond to what’s needed. Do encourage your child to use the young people’s form if they can.
