Early Intervention Panel

Supporting successful transitions for vulnerable learners

 

The Early Intervention Panel is a multi-agency forum designed to support vulnerable children with SEND at key points of transition, ensuring they have the right support to thrive in secondary school.

 

Held twice a year, the panel brings together expertise from across Southwark, including the SEND Consultant Team, Children’s Social Care, Educational Psychology, Youth Justice, SENDIPS, and the Inclusion and Monitoring Team.

  • Spring Term: Primary schools can refer children with SEND who may be vulnerable during the move to secondary school. Identified pupils will receive targeted transition support throughout the summer term to help them settle successfully into Year 7.
  • Autumn Term (Year 7): Secondary schools can refer children with SEND who are experiencing difficulties settling or have not transitioned well. The panel works collaboratively to explore holistic, multi-agency support to meet their needs.

This panel is part of Southwark’s commitment to inclusive education, ensuring that no vulnerable learner is left without coordinated, timely support during key transition periods.
