You have evidence that best practice has been applied and existing resources used but concerns persist. 

  
 

Two

Application for funding discussed with parents and parental consent requested to begin an application via the Provider Portal (consent is requested via Docusign). Consent received and application started.

  
 

Three

Application form completed with supporting information and sent to parent for final consent. Parent consents and application is submitted. 

  
 

Four

Application is received and allocated to the next available panel 

  
 

Five

EYIF panel meeting held and decision on funding made
