Holding an EHCP annual review

Southwark Council has worked alongside educational settings, support services and the parent carer forum to update the forms associated with statutory processes.

 

There are two versions of the annual review form – one is intended for the Early Years through to Year 4, the other from Year 5 onwards. It is expected that settings will use the new forms as soon as they are able. However, as schools may still be completing some statutory processes on older forms, Southwark SEN will accept the old forms until 1 April 2026. After this date, any annual reviews submitted with the old forms will likely require additional follow-up with settings.

When a child or young person is granted an EHCP the educational setting should continue to track the impact of provision using the Post EHCP Graduated Response form.

From 1 September 2025 we will be using a new process for submitting Annual Review documentation. Education settings are expected to arrange the annual review with the young person and their family, with SEND, social care and health professionals providing advice and input at least two weeks before the meeting.

After the Annual Review is held the full report needs to be circulated to all attendees within two weeks of the meeting and submitted to the SEND team: ehcp.annual.reviews@southwark.gov.uk

A decision will be made about the current EHCP within 4 weeks of the annual review.
